Your first visit will include your physical therapy evaluation, followed in most cases by your initial treatment. Your evaluation will include a thorough discussion of your past medical history, history of current condition for which you are coming to physical therapy, discussion of the impact your current condition has on your day to day and recreational activities, a physical examination to determine impairments, functional limitations, and cause of your current complaint, and a determination of your goals for attending therapy and a treatment program to achieve those goals.

Upon completion of your initial evaluation, frequently we will begin with the treatment program that was established. This may include a combination of hands on treatment, education, exercise, and other modalities to help decrease pain, reduce impairment, and restore function. In addition part of your first visit may include the introduction of some home exercises/treatment recommendations to assist in your rehabilitation. As part of your treatment program, your Physical Therapist will discuss the frequency (number of times per week) and duration (number of weeks), in their professional opinion, are required to achieve your goals.

Generally the initial visit will last about 75 minutes. We ask that you arrive 15 minutes prior to your scheduled time to complete paperwork. You will need to bring your prescription from the referring physician (and when necessary your PCP referral), your insurance card(s), and a photo ID for your first visit. In the case where the patient is a minor, a parent or legal guardian must accompany him/her for their first visit. In addition, it may be beneficial to bring clothing that is loose fitting and comfortable to move in.

We try at Maximum Performance Physical Therapy to be sensitive to each person's beliefs, and/or special needs. Please be sure to discuss any special requirements you may have with our front desk staff at the time you set up your initial appointment. We will make every effort in our capability to accommodate your requests.

Co-pays and Co-insurance payments will be due at the time of check-in for your first appointment. Frequently there will be payment necessary at the time of each visit. Please verify your benefits with your insurance provider, and feel free to ask our staff if there are any questions regarding your financial responsibility.
