General Information for Referring Physicians/Practitioners
Our staff at Maximum Performance Physical Therapy works hard to try to make the transition from the Physician/Practitioner office to our office as smooth as possible. It is our policy to educate patients during their initial call with regards to prescriptions and referrals that might be required by their insurance company prior to their first visit. It is our hope that by doing so we can eliminate confusion and minimize the need for unnecessary or last minute calls to your office for referrals. It is our policy to offer an initial appointment within 48 hours of the patientís initial call. We begin at 7am and accept our last appointment at 7pm to accommodate your patientís busy schedules.
It is also our policy to effectively communicate with our referring providers all necessary documentation with regards to patients initial appointments, progress notes before scheduled follow up appointments with the referring practitioner, and discharge status. Additionally, if there are extenuating circumstances that affect the patientís course of physical therapy, it is our policy to update the referring practitioner immediately. We feel effective communication is an important factor in providing the highest level of care for your patients.
